Prior to your consultation appointment you should have received an invitation to complete patient registration and health history forms. If you have not received this invitation, either by email or text message, please call our office to have the invitation sent to you. Likewise, you may use the Patient Registration portal on this website to access the registration forms.

If you do not have access to either email or text messaging, we are able to send you the necessary paperwork through the U.S. Postal Service if we have enough time to get the envelope to you before your first visit. You may return the completed forms by mail, drop them off at our office during office hours, or bring them with you at the time of your consultation appointment.

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Dr. Burns will perform an examination of your oral condition, answer any questions you may have about the condition and options available to you, and develop a treatment plan for your dental needs. The administrative team will investigate any insurance coverage that might be available to you and present your cost estimate for your treatment at this time. Payment arrangements will be made at this time for your convenience.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip provided to you by your referring dentist and any x-rays, if applicable;
  • A list of medications you are presently taking;
  • If you have medical or dental insurance, bring the associated insurance cards. This will save time during the consultation visit and allow us to help you calculate treatment estimates and process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian to the consultation visit.

If your dentist has taken x-rays of the area to be treated, please ask the dental office to forward them to our office. Digital x-rays may be emailed to our office, and physical x-rays or CDs/thumb drives may be mailed to us through the U.S. Postal Service.  If there is not enough time for the U.S. Postal Service to deliver them, please pick them up and bring them to our office. If additional films are necessary, we, will take those images at our office.

After consultation with Dr. Burns, the timing for treatment will be determined and based on your unique situation, including the following options:

  • Treatment that utilizes local anesthesia and nitrous oxide sedation may be performed on the same day as the consultation.
  • Complex medical history and/or treatment plan may require, first, a consultation and evaluation appointment, and a second, separate appointment to provide the agreed upon treatment on another day.
  • Procedures performed utilizing intravenous (IV) sedation always require two appointments: a consultation visit and a treatment visit.
  • At the end of your consultation, you will have a much better understanding of your dental needs, the process of treatment, and when your treatment will be performed.
