To begin the process, we need information from you before your appointment date so that we can be prepared to serve you in an efficient manner and focus on your dental needs.
Please use the following links to enter our patient portal for your registration information and health history information. The portals are mobile-friendly, should it be easier for you to complete the paperwork on your smartphone.
Radiographs/images: If you were given radiographs by your dentist, please be sure to bring them with you to our office.
Likewise, if your dentist was to mail or email the radiographs to us, please check with your dental office to be sure they were sent. If you have dental or medical insurance, please bring your card(s) and/or insurance data to your appointment as well.
We are located on the west side of US 33 in the Dunlap area of Elkhart, just north of Mishawaka Road. The office is a gray brick two-story building with a rectangular dark sign that shows our logo and practice name at the roadside. We are approximately one-half mile south of the Concord Mall on the same side of the street.
