Ways to Schedule an Appointment for your Patient:
- Your office is welcome to call us while your patient is present to schedule their appointment with us; or
- Your patient is welcome to call our office at their convenience to schedule the appointment.
Ways to Submit a Referral Form and Images to Our Office:
- The interactive version of our Referral Form will allow you to complete the form online, attach images, and submit all information to our office via secure, HIPAA-compliant portal;
- Or you may click on the images below to access a blank Referral Form and Patient Information Sheet that can be printed, completed, and either scanned/emailed, mailed through the U.S. Postal Service, or sent with your patient.
- If using email, please send completed form with any images you wish to share with us to our office via secure online portal.
Please send home with your patient the Patient Information Sheet to aid them in preparing for their visit to our office.
